Traditional & Alternative Rehab & Detox Programs Explained

What’s the difference between one rehab program and another? Which drug and alcohol detox and rehab program should you or a loved one go to? Do you know the different types or models of recovery?

Here is a summary of the different  detox and rehab programs that I am familiar with after working in the field of drug abuse for 10 years as a drug rehab consultant and drug prevention specialist.

Medical Detox

Medical detox is the use of medications, such as benzodiazepines, clonidine, suboxone, etc, to more smoothly withdraw someone from alcohol, methadone, opiates or other drugs of abuse. Approved medical facilities and medical doctors and staff are required to administer the various treatments to assist with difficult or otherwise long withdrawals from these drugs.

Some medical detox facilities use a combination of medical and natural / holistic approaches to detox. These methods generally speed recovery time and comfort through withdrawal according to client testimonials.

Non-Medical Detox

This might mean withdrawing someone from drugs without medication. Non-medical detox is sometimes more effective than medical detox as a drug abuser is sick of using drugs and is highly averse to any more drugs going into their bodies. Non-medical detox may mean the highly skilled use of alternative nutrients, sometimes IV’s and natural therapy’s to assist a person to more comfortably withdrawal and detox from drugs / pharmaceuticals and correct brain function. Some medical and non-medical detox is highly sophisticated and involves many testing and evaluation protocols not used by traditional medical detox centers. IE nutrient, vitamin and allergy testing to name only a few.

Bio-Physical Detox

Drug residuals have been found to get lodged in the fatty tissues of the body. This includes the brain, kidney, liver, muscles as well as the fat. Former drug abusers sometimes relapse back to using drugs due to physical cravings for the drugs which they say doesn’t go away after medical detox or rehab.

Bio-physical Detox involves a total and full detoxification requiring weeks or months of natural detox methods like extensive sauna baths, vitamin and nutritional supplementation, intensive exercise, proper rest, improving diet, drinking lots of water, etc.

Some Bio-physical Detox has been given extensive scientific testing and proven itself under many different populations – drug addicts; hazardous chemical / gas victims (fire fighters, pesticides / herbicides — DDT; mental / spiritual betterment, health improvement. Traditional & 12 Step Rehab Methods

Traditional rehab and 12 Step models comprise about 95% of the rehab facility programs in the US and Canada. Their successes vary massively depending on the center and how the center defines success. One center can vary greatly from another in terms of professionalism, care and results.

12 Step programs attempt to intervene between the decisions to abuse drugs and desires to stay clean. A drug addict or alcoholic have been solving life’s problems with a fix, drink, hit, or other drugs. 12 Step methods use sponsors, meetings, actions, education, etc techniques to stop further drug abuse. 12 Step programs also resemble social educational programs from step 4 onward, as a personal responsibility in ones recovery is expected of a 12 stepper. 12 Step includes principals of spiritual connection through-out the process.

Medical / psychological rehab programs use different drugs and counselors in their intervention efforts. Usually medical programs are either trying to make a client more comfortable while in rehab or they have actually done some extensive testing to find underlying medical problems which could by spurring the substance abuser to use drugs. A typical example is hypo-thyroid conditions. Psychological approaches either attempt to correct aberrant behavior through adjusting the thinking of the client. It may involve on-going counseling for in-determinant amounts of time after a stay in rehab is completed.

Sometimes drugs that create aversion to drinking are used as with Schick Shadel programs. Other substitute drugs like methadone for opiate abuse and Ritalin for cocaine / meth abuse have been used by many program models.

Non-Traditional Rehab Methods

Social Educational or learning models focus on improving abilities and increasing self control or will power through enlightenment or cognitive realization. Combinations of study, exercise, counseling and practical application of processes are applied to bring about desired states of being. Often a type of therapeutic community is established due to the camaraderie of clients assisting each other through the tough times of their recovery.

As social educational programs are my forte, I will list the specific programs I am familiar with along with their posted success rates.

Jude Thaddeus Program (JT), or St. Jude’s is a social/educational approach rather than a medical/psychological methodology putting control over one’s recovery in their own hands rather than a doctor, psychologist, or group. JT provides an opportunity to permanently recover, because JT believe’s it is a choice – not a disease. JT’s success rate for 12 years is over 68%. (Ref: St. Jude Retreat House)

Rational Recovery or RR, has a different approach than traditional or 12 Step methods. RR labels different behaviors and parts of a mind model to get a person to recover through cognitive or increased awareness and control over their addictions. Focus is on self-recovery from addiction to alcohol and other drugs through planned, permanent abstinence through greater self awareness / control. (Ref: Jack Trimpey, Founder RR)

Narconon (NN) Drug Rehabilitation is a drug-free all natural social educational approach to ending addiction. Narconon considers clients as students who are not trying to recover from an illness. Working in the drug rehab / addictions and detox field for 10 years, I’ve discovered that there is always more than one answer. Problems as life threatening that people fall fully into drug dependency or abuse are very major problems.

Finding the Right Rehab Program

The right questions should be: How long have you been abusing drugs or alcohol? What happens when you try to stop? How long can you stay clean? Do you have something in your life that you could make more desirable than drugs? Do you know how to make your body function normally without drugs?

People get trapped into using and abusing alcohol and drugs because they really don’t know any better. If they had a better solution to their problems they would do that. But all they’ve got is a bottle, a prescription or some drugs to solve their problems.

Take a different approach to the type of program or methods you’ve already tried. That means if the rehab or detox programs were a drug substitute or psych based program, try alternative or holistic. The idea here is that if the method of rehab or detox hasn’t been working, use a different method.

Here is a detox and rehab success story from a person who simply stopped doing the ‘usual’ and tried a different approach than the ones he’d already tried, 10 times before.

I have been sober for about 3 years now.  I have a job, I have a good relationship with my family, and I am happy for the first time in years.

I had been in over 10 different rehabilitation centers, some of which I had gone back to numerous times.  My family was in total support of me getting clean but would not support me in my drug addiction which left me homeless for much of the time that I was using.  Every treatment center that I went to wanted to free me from drug addiction and deliver me to my family a sober individual.  I was put on medications that temporarily solved my problem, but once I was off of them I would find myself back on drugs and unable to deal with my life, some of the medications I was given gave me more problems than I originally had.

Restoring self esteem and personal integrity is an action done while in rehab. Inpatient rehab centers have the task of helping and encouraging clients to fully regain self respect.

What rehabs don’t work

Rehab centers and detox centers are not created equally. Many rehab programs are simply working out of a hospital or riding off of insurance claimants attending their program. These centers usually won’t post success rates. They teach clients that “relapse is part of recovery” as a method of excusing ineffective treatments.

Recovery and Rehabilitation should be synonymous. Sending addicts home before they are fully detoxified and rehabilitated is the cause of most rehab failure. If a person is still ‘recovering’ then they haven’t recovered, have they?

Factually, there are social fellowships which aid and contribute to ones sobriety. These fellowships and organizations are wonderful. But social fellowships are for those who are clean and sober. My point is that rehab programs have a start and a finish. Rehab is complete after recovery is achieved.

Low cost half way house

Therapeutic communities can offer wonderful support to a person recovering from a long lifestyle of addiction. Low cost ½ Way Houses are one way to transition back to responsible living in society.

Some rehab programs expect transition into a ½ Way House after a rehab program. Other rehab centers expect the person to have achieved a returned level of responsibility by the time they complete a rehab program.

Therefore a low or lower cost half way house is intended to either transition a person from rehab to real life, or confirm that a rehab program was effective before moving back into society. People who have no or few work related skills should consider transitional housing as part of a rehab plan.

Family members need objective information about effective rehab centers.

Drug addiction and alcoholism disrupts and torments families as well as ruining the abuser’s life. When a family member truly wants the best recovery program for their loved one, a drug rehab consultant should be contacted.

Factually, select rehab options get 65% to 94% success on average. Higher success would be possible but some drug abusers don’t wish to end their habit. By my experience approximately 80% of addicts going to rehab want to end their addiction. But, they simply don’t know how to move on in life and stay clean.

You may have your own blind spots regarding a family member’s addiction. Having an outside and unbiased consultant consider factors you haven’t yet looked at can make a huge difference in selecting a successful program the first time or the tenth time.

Determining the potential success of a detox or rehab candidate is part of a consulting assessment.

Alcohol and drug treatment is most effective in inpatient recovery centers for longer term problems.

Drug rehab should be the process of riding an addict or alcoholic of obsessions and compulsions to abuse drugs. Effective substance abuse treatment programs should return a teen or adult to full control over their lives again, not merely get the person to stop drinking or drugging.

Outpatient Programs

Rehab programs that use counselors, doctors, social workers, meetings etc, while the person continues living at home is effective for people able to stop using drugs and alcohol while processing their lives through recovery.

If a person is ’slipping’ frequently or continue to use, outpatient programs are not appropriate for their situation.

Outpatient programs are generally run through state or city funded health centers, community centers, or private counselors.

Inpatient Drug and Alcohol Rehab

An inpatient program may be located in a private residence, lodge, center, clinic, hospital or retreat.

1. Publicly funded or partially subsidized inpatient rehab is usually run through centers or clinics designed specifically for a rehab program. Some are simply add-ons to a hospital or clinic. These should provide space to exercise, take a walk and also be in a very secure location.

The philosophy of the center should focus on complete rehabilitation, not simply beginning a continuum of care.

2. Private rehab residences, centers or retreats are generally located in pleasant and therapeutic locations that assist recovery. Sometimes exclusive residential rehab centers are required for individuals needing individual attention or privacy.

The methods of rehabilitation used should be consistent with their philosophy. Are medications used at the center? Have they compromised their program philosophy by using mood altering or addictive drugs? Do they expect their clients to recover at the center?

3. Larger drug rehab centers also offer opportunities of a therapeutic community, lots of space and amenities, exercise areas etc, all on the premises. Large rehab centers provide a different experience than those exclusive to a handful of clients.

How many trained counselors are at the center? Do they do security watches? How do they respond to drug or alcohol violations?

Referring clients to rehabs that work is the first priority and commitment made to a client. This may mean a rehab consultant refers substance abusers to treatment in more modest or more expensive, but secure surroundings. It may require they travel far away to get to a safe and suitable location.

Inpatient facilities tend to work best for teens and addicted people who wish to get assistance and resolve a drug problem rapidly and completely.

4. Luxury rehab centers and detox facilities have made an inpatient program more comfortable for substance abuse treatment. The finest effective rehab may require anticipating all of a clients needs. Luxury accommodations and holistic medical care offer the finest in recovery programs. These are not necessarily the most effective, but may provide services beyond the norm. Essentially, people’s problems must be met through the rehab program.

The Purpose of Drug & Alcohol Rehabilitation

Sober living is the primary objective of rehab. Insuring successful sobriety requires something more from a rehab. Health, honesty, success and happiness of a person returning to live in society should be the method of assuring long-term recovery.

Therefore finding and locating drug and alcohol rehab centers that use effective programs of recovery, should be the objective of a family member seeking help for a loved one abusing drugs or alcohol.

Rehabilitate means to bring back to proper or a former healthy or good condition. If a person still needs treatment, have they been rehabilitated?

Rehabilitation includes returning social responsibility to substance abusers as well as repairing the physical and mental consequences of drug abuse. Emotionally, people may be a wreck after drugs have ravaged their lives. All these factors should be addressed in a holistic way. That would make up a recovery program worthy of the term rehabilitation.

Drug rehab programs should insist the person in recovery demonstrates they have been rehabilitated before leaving the rehab center. Rehabilitation happens in rehab, and evidence of recovery through ones actions, responsibility and self control is manifested before returning home.

Caution:

If a rehabilitation programs don’t or won’t clearly relay success rates and their in-patient treatment is 30 days, a person may expect many return visits and still not achieve full recovery.

Relapse doesn’t need to be part of a recovery plan. However, knowledge of slips and how to repair oneself if they occur should be included in a life-long strategy.
